Population Health Registered Nurse

📋 Description

• Telephonic post-discharge outreach to assess symptoms and barriers
• Educate and coordinate preventative health screenings
• Manage chronic disease and medication adherence education
• Discuss Galileo's care model with patients
• Coordinate care across hospital/ED, pharmacies, and providers
• Understand health plan contracts and Galileo markets
• Meet performance targets as an individual contributor
• Collaborate with Engagement and Population Health leadership to improve outcomes

🎯 Requirements

• 2+ yrs in case mgmt (insurance or VBC) or PCP RN in population health
• Experience with Medicare/Medicaid populations with complex chronic conditions
• Nurse Licensure Compact (NLC) required
• Active RN license(s) in NY/NV/MI/MN/OR/CA/MA/CT/IL (a plus)
• Motivational interviewing to encourage behavioral changes
• Excellent written and verbal communication skills
• Familiarity with Population Health and HEDIS gaps in care
• Mac OS and Google Suite proficiency
