Population Health Nurse Practitioner (Remote, CT License)

📋 Description

• Cover care needs for the Transitions of Care program.
• Telephonic outreach and scheduled video/phone visits post-discharge.
• Focus on med reconciliation, symptom screening, follow-up orders.
• Build trust, explain Galileo's care model, connect to services.
• Coordinate care across hospital/ED transitions, pharmacies, and community providers.
• Collaborate with our nursing team.
• Be accountable to individual targets while contributing to team population health goals.

🎯 Requirements

• Certified Nurse Practitioner in Adult, Family, or Geriatrics
• Active MA or CT licensure (ability to obtain others)
• 5+ years post-graduate clinical experience in Primary Care or Geriatrics
• 2 years may be substituted by time in Skilled Nursing or Long-Term Care
• Direct experience engaging Medicare and Medicaid populations with complex conditions
• Motivational interviewing skills
• Comfort with digital-first environment and virtual care delivery
• Proficiency with Google Suite and Slack
• Excellent written and verbal communication skills
• Bilingual English/Spanish (bonus)
