- Conduct in-home visits for patients in private homes and facilities.
- See patients in Chronic Care Management Centers in person or via Telemedicine.
- Perform clinical assessments, diagnose conditions, and develop care plans.
- Perform X-rays and imaging of lower extremities as needed.
- Advise on foot care, prescribe orthotics and pain relief.
- Coordinate care with PCPs and other clinicians to monitor progress.
🎯 Requirements
- Current AZ license in good standing; willing to obtain if needed.
- Board eligible/certification in Podiatry.
- Two to four years experience as a Podiatrist preferred.
- Excellent clinical, teaching, interpersonal skills; able to evaluate outcomes.
- Proficient with computer, laptop, tablet and smartphone for charting and telemedicine.
- Must have current DEA license.
