📋 Description

• Provide frontline care to unhoused patients in field clinics and shelters.
• Deliver acute, chronic, preventive, and palliative care in the street setting.
• Collaborate with nurses, social workers, and outreach staff to coordinate care.
• Design and implement care plans toward stability and better outcomes.
• Document encounters to maintain up-to-date medical records.
• Mentor other care team members and support clinical workflows.

🎯 Requirements

• 2+ years of clinical experience caring for underserved populations.
• Street medicine or community-based care experience preferred.
• Active CA Physician and Surgeon license.
• Board certification or eligibility (see below).
• DEA registration or eligibility.
• NPI number and BLS certification.
