Collaborate with a multidisciplinary care team to coordinate care.
Design care plans to improve stability and outcomes.
Document encounters for continuity of care.

🎯 Requirements

2+ years clinical experience serving underserved populations.
Experience in street medicine or community-based care.
Familiarity with Kern County medical and social resources.
Comfort providing direct care in outdoor field settings.
Strong interpersonal and communication skills.
Ability to travel up to 75% with CA driver's license.
