Physical Therapist - Senior Living Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements of mobility, ambulation, strength, balance, and safety.
• Implement wound care and protective interventions with evidence-based dressings/topical agents.
• Develop and implement individualized care plans (manual therapy, gait training, balance).
• Educate clients and families in rehabilitative care for health, safety, and independence.
• Accurately document observations, interventions, and evaluations via a touch pad tablet.

🎯 Requirements

• Current OH PT license.
• Minimum 1 year PT experience.
• Graduation from an APTA-approved PT program (transcript/diploma).
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
