  • Make home visits to clients in designated territories.
  • Perform diagnostic tests and measurements (mobility, ROM, balance, safety).
  • Implement integumentary interventions to prevent wounds and promote healing with evidence-based dressings.
  • Develop and implement individualized care plans, including manual therapy and gait training.
  • Continually assess and revise the PT care plan; coordinate with nursing on multidisciplinary team.

🎯 Requirements

  • Current North Carolina PT license.
  • Minimum 1 year of PT experience.
  • Graduation from an APTA-approved or AMA-accredited program.
  • Ability to work independently and manage time.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Proficient with computers; EMR experience preferred.
