📋 Description

• Make home visits to clients in designated territories.
• Assess mobility, transfer status, balance, strength, and safety.
• Apply wound care and preventive dressings with evidence-based methods.
• Create and revise individualized PT care plans; include gait and balance training.
• Educate clients and families on rehab activities to promote health and independence.
• Document care and evaluations in EMR.

🎯 Requirements

• A current OH Physical Therapist license.
• A minimum of one year of recent work experience as a Physical Therapist.
• Graduation from a program approved by APTA/CAAHEA/AMA as indicated by transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; prior experience with EMR preferred.
