📋 Description

• Make home visits to clients in designated territories.
• Perform diagnostics: mobility, transfers, balance, strength, endurance, safety.
• Implement wound care with evidence-based dressings and topical agents.
• Develop and adjust PT care plans including manual therapy and gait training.
• Educate clients and families on rehabilitative care for health and independence.
• Document observations and interventions using a touch pad tablet.

🎯 Requirements

• North Carolina Physical Therapist license.
• Minimum of one year of PT experience.
• Graduate from APTA- or CAAHEA-accredited PT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
