Physical Therapist, (PT) - Home Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements: mobility, gait, strength, balance.
• Implement wound care with evidence-based dressings.
• Develop and implement individualized PT care plans with manual therapy, gait, balance.
• Continuously assess and revise care plans with nursing in the multidisciplinary team.
• Educate clients and families on rehabilitative care and safety.
• Document observations and interventions using a touch-pad tablet.

🎯 Requirements

• North Carolina PT license.
• Minimum 1 year PT experience.
• Graduate of an APTA-approved PT program.
• 2-3 days/week availability required.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
