Physical Therapist, PT, Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Assess mobility, ROM, gait, balance, strength, and endurance.
• Implement wound care with evidence-based dressings.
• Develop individualized care plans including manual therapy and gait training.
• Assess and revise PT care plan; coordinate with nursing.
• Educate clients and families on rehab to promote safety and independence.

🎯 Requirements

• PA Physical Therapist (PT) license.
• Minimum 1 year of PT experience.
• Graduate of an APTA-approved program or AMA-accredited school.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
