Physical Therapist, PT, Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests: mobility, ROM, balance, gait, strength, endurance, safety.
• Apply wound care using evidence-based dressings and topical agents.
• Develop individualized care plans with manual therapy, gait and balance training.
• Assess and update PT care plan with the nursing team.
• Educate clients and families on rehabilitative care and safety.
• Document observations and care using a tablet-based EMR.

🎯 Requirements

• Current PA Physical Therapist - PT license.
• Minimum one year of PT experience.
• Graduate from an APTA/CAHEA/AMA-approved program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior EMR experience preferred.
