Make home visits to clients in designated geographic territories.
Perform diagnostic tests and measurements: mobility, ROM, transfers, balance.
Implement wound care with evidence-based dressings.
Develop and implement individualized care plans (manual therapy, gait, balance).
Continually assess and revise PT care plan with nursing.
Educate clients and families in rehabilitative care.

🎯 Requirements

PA Physical Therapist (PT) license.
Minimum 1 year PT experience.
Graduate of an APTA-approved/AMA-accredited program.
Ability to work independently and manage time.
Strong interpersonal, organizational, and problem-solving skills.
Solid computer skills; EMR experience preferred.
