Physical Therapist (PT) - Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Perform diagnostic tests and measurements: ROM, transfers, balance, gait, strength.
• Implement wound care using evidence-based dressings/topical agents.
• Develop and implement individualized care plans (manual therapy, gait training, balance).
• Continually assess and revise the PT care plan and collaborate with nursing in multidisciplinary care plans.
• Educate clients and families in rehabilitative care to promote health, safety, and independence.

🎯 Requirements

• A current NJ Physical Therapist license.
• Minimum one year of PT experience.
• Graduation from an APTA or AMA-approved program (transcript/diploma).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.
