Physical Therapist, PT, Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Perform mobility and strength assessments, gait and balance checks.
• Implement wound care and evidence-based dressings to promote repair.
• Develop and implement individualized care plans with manual therapy and training.
• Continually assess and revise PT care plans with nursing input.
• Educate clients and families on rehab and safety to promote independence.

🎯 Requirements

• A current PA Physical Therapist license.
• At least 1 year PT experience.
• Graduate from CAPTE-approved PT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
