Physical Therapist, PT, Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform tests and measurements on mobility, ROM, transfers, balance, and safety.
• Implement wound care and dressings to promote healing.
• Develop individualized care plans including manual therapy, gait and balance training.
• Educate clients and families in rehabilitative care to promote health and independence.
• Document observations, interventions, and evaluations using a touch pad tablet.

🎯 Requirements

• Current PA Physical Therapist (PT) license.
• Minimum one year of PT experience.
• Graduation from an APTA-approved program (with transcript/diploma).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior EMR experience preferred.
