  • Make home visits to clients in designated geographic territories.
  • Perform diagnostics: mobility, ROM, stability, gait, strength, balance, safety.
  • Implement wound care and protective interventions with evidence-based dressings.
  • Develop individualized care plans including manual therapy, gait training, balance.
  • Assess and revise the PT care plan; collaborate with nursing in the multidisciplinary plan.
  • Educate clients and families in rehabilitative care to promote health, safety, independence.

🎯 Requirements

  • North Carolina PT license.
  • At least 1 year PT work experience.
  • Graduation from an APTA-approved program (or CAHEA/AMA) with transcript.
  • Ability to work independently and manage time.
  • Strong interpersonal, organizational and problem-solving skills.
