- Make home visits to clients in designated geographic territories.

- Assess and revise the PT care plan; collaborate with nursing in the multidisciplinary plan.
- Educate clients and families in rehabilitative care to promote health, safety, independence.
🎯 Requirements
- North Carolina PT license.
- At least 1 year PT work experience.
- Graduation from an APTA-approved program (or CAHEA/AMA) with transcript.
- Ability to work independently and manage time.
- Strong interpersonal, organizational and problem-solving skills.
