Physical Therapist, PT, Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform mobility and strength assessments, balance, and safety checks.
• Provide wound care with evidence-based dressings and topical agents.
• Develop care plans incl. manual therapy, gait training, balance.
• Continually assess and revise the PT care plan with nursing in the multidisciplinary team.
• Educate clients and families in rehabilitative care to promote health, safety, and independence.

🎯 Requirements

• PA Physical Therapist - PT license.
• At least 1 year of PT experience.
• Graduate from an approved PT program (APTA or AMA CAAHEA).
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Strong computer skills; EMR experience preferred.
