Physical Therapist, (PT) - Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Perform diagnostic tests: mobility, ROM, balance, endurance.
• Implement wound care with evidence-based dressings.
• Develop individualized care plans: manual therapy, gait, balance.
• Evaluate and revise PT care plans; coordinate with nursing.
• Educate clients and families to promote independence.

🎯 Requirements

• A North Carolina Physical Therapist license (PT).
• Graduation from an APTA-approved program (transcript/diploma).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, problem-solving skills.
• Solid computer skills; EMR experience preferred.
