Physical Therapist, PT, Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Perform diagnostic tests and measurements of mobility, gait, strength, balance, and safety.
• Implement wound care using evidence-based dressings.
• Develop individualized care plans, including manual therapy, gait training, balance.
• Continually assess and revise the PT care plan with the team.
• Educate clients and families in rehabilitative care to promote independence.

🎯 Requirements

• Current PA Physical Therapist (PT) license.
• Minimum one year PT experience.
• Graduate from APTA-approved program or AMA-accredited.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
