Physical Therapist, PT, Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform mobility/ROM, transfers, balance and safety assessments.
• Implement wound care to prevent wounds and promote healing.
• Develop and implement individualized care plans with gait, balance, and manual therapy.
• Educate clients and families on rehabilitative care and safety.
• Document care observations and interventions on a tablet.

🎯 Requirements

• A current DE Physical Therapist - PT license.
• Minimum of one year of PT experience.
• Graduation from a CAAHEA/APTA-approved program with transcript/diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
