Physical Therapist, PT, Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Perform diagnostic tests: ROM, gait, strength, balance.
• Implement wound care and dressings to promote healing.
• Develop and revise care plans: manual therapy, gait, balance.
• Educate clients and families in rehabilitative care.
• Document observations and interventions in EMR.

🎯 Requirements

• A current PA Physical Therapist - PT license.
• A minimum of one year of recent PT experience.
• Graduation from an APTA-approved program or AMA CAAHEA accreditation.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.
