Physical Therapist, PT, Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements on mobility, ROM, balance, strength, and safety.
• Implement integumentary protective interventions to prevent wounds and promote healing with evidence-based dressings.
• Develop and implement care plans, including manual therapy, gait training, balance.
• Assess and revise PT care plan; participate with nursing in multidisciplinary care.
• Educate clients and families in rehabilitative care and activities to promote health and independence.

🎯 Requirements

• PA Physical Therapist license (current).
• Minimum of one year PT experience.
• Graduate from an APTA/AMA-accredited PT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, problem-solving skills.
• Solid computer skills; EMR experience preferred.
