Physical Therapist, PT, Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements: ROM, strength, balance, safety.
• Implement wound care to prevent and promote healing.
• Develop care plans including manual therapy, gait training, balance.
• Educate patients and families on rehabilitative care and safety.
• Document observations and interventions in EMR on a tablet.

🎯 Requirements

• A current PA Physical Therapist license.
• Minimum of one year of PT experience.
• Graduation from a program approved by the APTA or CAAHEA.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.
