Physical Therapist, PT, Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, strength, balance, safety).
• Implement wound care and protective interventions with dressings.
• Develop and implement individualized PT care plans (exercises, gait, balance).
• Assess and revise PT care plan with the multidisciplinary team.
• Educate clients and families; document observations and interventions using a tablet.

🎯 Requirements

• A current PA Physical Therapist - PT license.
• Minimum of one year of recent PT experience.
• Graduation from an APTA- or AMA-accredited PT program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.
