Physical Therapist, (PT) - Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Perform mobility, range of motion, stability, and safety assessments.
• Provide wound care and dressings to promote healing.
• Develop individualized care plans with exercises and gait/balance training.
• Revise care plans with nursing and the multidisciplinary team.
• Educate clients and families on rehabilitative care and safety.

🎯 Requirements

• NC Physical Therapist license.
• Degree from an APTA/CAHEA/AMA-approved program (transcript or diploma).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
