Physical Therapist, PT, Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Conduct diagnostic tests and measurements of mobility, ROM, balance, and endurance.
• Implement wound care interventions using evidence-based dressings and topical agents.
• Develop and implement individualized care plans, including manual therapy and gait training.
• Continually assess and revise PT care plans; coordinate with nursing in MDT care plan.
• Educate clients and families in rehabilitative care to promote health and independence.

🎯 Requirements

• PA Physical Therapist (PT) license current.
• Minimum one year of recent PT experience.
• Graduate of CAPTE-accredited PT program (transcript/diploma).
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
