Make home visits to clients in designated territories.
Assess mobility, ROM, strength, balance, safety.
Implement wound care and dressings per evidence-based protocols.
Develop and update individualized PT care plans; coordinate with nursing.
Educate clients and families on rehabilitative care for safety and independence.
Document observations and interventions using EMR/tablet.

🎯 Requirements

A current North Carolina PT license.
Graduate of an APTA-approved PT program; transcript/diploma required.
Ability to work independently and manage time effectively.
Strong interpersonal, organizational, and problem solving skills.
Solid computer skills; EMR experience preferred.
