📋 Description

• Make home visits to clients in designated geographic territories.
• Perform mobility, ROM, transfer, strength, and balance assessments.
• Implement wound care using evidence-based dressings and agents.
• Develop and adjust individualized care plans with PT interventions.
• Educate clients and families to promote safety and independence.
• Document observations and evaluations with a state-of-the-art EMR.

🎯 Requirements

• Current North Carolina Physical Therapist (PT) license.
• Graduate of an APTA-approved program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficient with electronic medical records (EMR).
