Make home visits to clients in designated territories.
Perform diagnostic tests and measurements (mobility, ROM, balance, strength).
Implement wound care with evidence-based dressings.
Develop and implement individualized care plans (manual therapy, gait, balance).
Educate clients and families in rehabilitative care.
Document observations and interventions using EMR tablet.

🎯 Requirements

Current NC Physical Therapist license.
Minimum 1 year PT experience; home health required.
Graduation from APTA-approved program (CAPTE).
Ability to work independently and manage time effectively.
Strong interpersonal, organizational, problem solving skills.
Solid computer skills; EMR experience preferred.
