Physical Therapist, PT - Home Health

📋 Description

• Make home visits to clients in designated territories.
• Test mobility, ROM, gait, strength, balance.
• Implement wound care with evidence-based dressings.
• Develop and adjust care plans including manual therapy.
• Assess and revise PT care plans; collaborate with nursing.
• Educate clients and families in rehab and safety.
• Document observations and interventions in EMR using touch tablet.

🎯 Requirements

• A North Carolina Physical Therapist license.
• Minimum of one year of PT experience.
• Graduate from an APTA- or AMA-approved PT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
