Physical Therapist, PT (Home Health)

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform mobility, ROM, transfer, balance, gait, and strength assessments.
• Implement wound care and protective interventions with evidence-based dressings.
• Develop individualized care plans with manual therapy and gait training.
• Assess and revise the PT care plan with nursing in the multidisciplinary team.
• Educate clients and families on rehabilitation and independent living.
• Document observations and interventions in EMR via touch-pad tablet.

🎯 Requirements

• A current DE Physical Therapist (PT) license.
• Minimum of 1 year of PT experience.
• Graduation from an APTA/CAAHEA-approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
