Physical Therapist, (PT) - Home Health

📋 Description

• Make home visits to clients in designated territories.
• Perform mobility, range of motion, transfer, strength, balance, and ambulation assessments.
• Implement wound care using evidence-based dressings and topical agents.
• Develop and revise individualized care plans, including manual therapy and gait/balance training.
• Document observations and interventions using a state-of-the-art tablet.
• Educate clients and families on rehabilitative care to promote health and independence.

🎯 Requirements

• Current North Carolina Physical Therapist license.
• Graduation from an APTA-approved program or AMA accreditation.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.
