- Make home visits to clients in designated geographic territories.

- Continually assess and revise PT care plan; coordinate with nursing in multidisciplinary plan.
- Educate clients and families in rehabilitative care to promote health, safety and independence.
π― Requirements
- A current North Carolina Physical Therapist license.
- A minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by APTA or AMA accreditation (transcript/diploma).
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills; EMR experience preferred.
