πŸ“‹ Description

  • Make home visits to clients in designated geographic territories.
  • Perform diagnostic tests and measurements (mobility/ROM, transfers, balance, strength, safety).
  • Implement wound care interventions with evidence-based dressings and topical agents.
  • Develop and implement individualized care plans including manual therapy, gait training, balance.
  • Continually assess and revise PT care plan; coordinate with nursing in multidisciplinary plan.
  • Educate clients and families in rehabilitative care to promote health, safety and independence.

🎯 Requirements

  • A current North Carolina Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by APTA or AMA accreditation (transcript/diploma).
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills; EMR experience preferred.
