Make home visits to clients in designated territories.
Perform mobility, ROM, transfer, balance and safety assessments.
Implement wound care using evidence-based dressings and topical agents.
Develop and update individualized care plans, including gait training and balance.
Coordinate with nursing in multidisciplinary care planning.
Educate clients and families on rehabilitation and safety.

🎯 Requirements

NJ Physical Therapist license.
At least 1 year of PT experience.
Graduation from APTA-approved program (or AMA/CAAHEA).
Ability to work independently and manage time.
Strong interpersonal, organizational, and problem-solving skills.
Proficient with computers; EMR experience preferred.
