Physical Therapist (PT)

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests: mobility, transfers, balance, strength, endurance.
• Implement wound care and dressings with evidence-based methods.
• Develop care plans: manual therapy, gait and balance interventions.
• Assess and revise care plans; collaborate with nursing.
• Educate clients and families on rehab and safety.
• Document observations on a state-of-the-art tablet.

🎯 Requirements

• Current NJ PT license.
• Minimum 1 year PT experience.
• Graduate of APTA/CAAHEA-approved program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational and problem solving skills.
• EMR experience preferred.
