Make home visits to clients in designated geographic territories.
Assess mobility, range of motion, strength, balance, and safety.
Implement care plans, manual therapies, gait training, and endurance activities.
Educate clients and families on rehabilitative care and independent living.
Accurately document interventions and evaluations using a touch pad tablet.
Collaborate with nursing and other clinicians as part of the care team.

🎯 Requirements

Active and current PT license.
One year of clinical experience preferred.
Previous home health care experience welcomed, but not required.
We will train you.
Ability to travel locally to cases as assigned.
