Physical Therapist (PT)

📋 Description

• Visit clients in their homes within designated territories.
• Provide PT care with 1:1 patient focus.
• Perform clinical assessments: mobility, ROM, strength, balance, safety.
• Create and update individualized care plans, including gait training.
• Educate clients and families on rehab and safety.
• Document observations and interventions with the electronic system.

🎯 Requirements

• Active PT license
• 1 year clinical experience preferred
• Home health exp welcomed, training provided
• Ability to travel locally to cases
