Make home visits to clients in designated territories.
Perform diagnostic tests: mobility, range of motion, gait, balance.
Implement wound care with evidence-based dressings and topical agents.
Develop and revise individualized PT care plans with manual therapy and gait training.
Continually assess and revise PT care plan with nursing in the multidisciplinary team.
Educate clients and families on rehabilitative care and safety and document observations with a tablet.

🎯 Requirements

Active and current PT license.
One year of clinical experience preferred.
Home health care experience welcomed, but not required. We will train you!
Ability to travel locally to cases as assigned.
