Make home visits to clients in designated geographic territories.
Assess mobility, ROM, gait, strength, balance, and safety.
Implement wound care and dressings to promote healing.
Develop and adjust individualized PT care plans, including exercises and gait training.
Educate clients and families to promote health and independent living.
Document care using a tablet and collaborate with the care team.

🎯 Requirements

Active and current PT license.
One year of clinical experience preferred.
Home health care experience welcomed, but not required. We will train you.
Ability to travel locally to cases as assigned.
