Physical Therapist - Per Diem, Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Assess mobility, ROM, strength, balance, and safety.
• Implement wound care with evidence-based dressings.
• Develop and revise individualized care plans (manual therapy, gait, balance).
• Educate clients and families on rehab activities and safety.
• Document care and interventions in EMR on a tablet.

🎯 Requirements

• Current MD Physical Therapist - PT license.
• Graduation from an APTA-accredited PT program (or AMA CAAHEA accredited).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
• Per diem/home health visits experience preferred.
