Physical Therapist, Part-Time

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform mobility, ROM, strength, balance, and safety assessments.
• Implement wound care with evidence-based dressings and topical agents.
• Develop and adjust individualized care plans (manual therapy, gait, balance).
• Continuously assess and revise the PT care plan with nursing.
• Document observations and interventions on a touch pad tablet.

🎯 Requirements

• Active PA Physical Therapist license.
• Home health experience welcomed; we will train you.
• Ability to travel to cases as assigned.
• Calm, reassuring presence with active listening and strong assessments.
• Excellent written and verbal communication.
• Deliver patient care with compassion, excellence, and reliability.
