Make home visits to clients in designated territories.
Perform diagnostic tests: mobility/ROM, strength, balance.
Develop and implement care plans: manual therapy, gait & balance training.
Assess and revise PT plan; coordinate with nursing in MDT care planning.
Educate clients and families in rehabilitative care for safety and independence.
Document care using EMR on a tablet.

🎯 Requirements

NJ PT license required.
Minimum 1 year PT experience.
Graduate from an approved PT program (APTA or AMA).
Ability to work independently and manage time.
Strong interpersonal, organizational and problem-solving skills.
Solid computer skills; EMR experience preferred.
