Make home visits to clients in designated geographic territories.
Perform diagnostic tests and measurements (mobility, ROM, balance, endurance).
Develop and implement individualized care plans including exercises and gait training.
Continuously assess and revise care plans with nursing as part of the team.
Educate clients and families on rehabilitative care for safety and independence.
Document observations and interventions using a tablet EMR system.

🎯 Requirements

NJ Physical Therapist license.
Minimum 1 year PT experience.
APTA-approved program or AMA accreditation.
Ability to work independently and manage time.
Strong interpersonal, organizational, problem-solving skills.
Solid computer skills; EMR experience preferred.
