- Make home visits to clients in designated territories.

- Provide wound care and dressings as prescribed.
- Develop and implement individualized care plans (exercises, gait, balance).
- Continually assess and revise PT care plan with nursing and the team.
- Educate clients and families on rehabilitation and safety.
- Document observations and treatments in the EMR.
🎯 Requirements
- Colorado Physical Therapist license.
- Minimum 1 year PT experience.
- Degree from an APTA-approved program.
- Ability to work independently and manage time.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills; EMR experience preferred.
