- Make home visits to clients in designated geographic territories.

- Continually assess and revise PT care plans with the multidisciplinary team.
- Educate clients and families in rehabilitative care to promote health, safety, and independence.
π― Requirements
- Pennsylvania PT license.
- Minimum one year of PT experience.
- Graduate from APTA/CAPTE approved program (transcript/diploma).
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem solving skills.
- Solid computer skills; EMR experience preferred.
