Physical Therapist - Home Health Visits (Full-Time)

📋 Description

• Make home visits to clients in designated territories.
• Assess mobility/ROM, transfers, stability, ambulation, strength, balance, safety.
• Implement wound care with evidence-based dressings and topical agents.
• Develop and revise care plans: manual therapy, gait, balance, interventions.
• Collaborate with nursing in the multidisciplinary care plan.
• Educate clients and families on rehab activities to promote independence.
• Document care using a touchscreen tablet for observations and interventions.

🎯 Requirements

• Current Pennsylvania Physical Therapist license.
• Graduation from APTA/CAHEA-accredited program; transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, problem solving skills.
• Solid computer skills; EMR experience preferred.
