Physical Therapist- Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Perform mobility, ROM, strength, balance and safety assessments.
• Implement wound care and prevention interventions with evidence-based dressings.
• Develop and adjust individualized PT care plans, including gait training.
• Continually assess and revise the PT care plan with nursing.
• Educate clients and families in rehabilitative care to promote independence.

🎯 Requirements

• Indiana Physical Therapist license.
• Minimum of one year PT experience.
• Graduation from an APTA- or CAPTE-approved Physical Therapy program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
