Make home visits to clients in designated territories.
Perform diagnostic tests and measures: mobility, ROM, strength, balance, safety.
Implement wound care using evidence-based dressings and topical agents.
Develop and implement individualized care plans (manual therapy, gait, balance).
Continually assess and revise the PT care plan; coordinate with nursing.
Educate clients and families on rehabilitative care to promote independence; document in EMR.

🎯 Requirements

Colorado Physical Therapist license.
Minimum one year of PT experience.
Graduation from a program approved by APTA or CAAHEA/AMA committee.
Ability to work independently and manage time effectively.
Strong interpersonal, organizational, and problem-solving skills.
Solid computer skills; EMR experience preferred.
