Physical Therapist - Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Perform mobility, gait, balance, strength, and safety assessments.
• Implement wound care and protective interventions using evidence-based dressings.
• Develop and update individualized care plans including manual therapy and gait training.
• Educate clients and families on rehabilitative care for health and independence.
• Document observations and interventions in the EMR using a touch pad tablet.

🎯 Requirements

• A Maryland PT license.
• Minimum of one year PT experience.
• Graduation from an APTA-approved program (or AMA accreditation) as indicated by transcript/diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior EMR experience preferred.
